The problem revolves around addressing obesity in patients like Sarah, a 38-year-old grappling with obesity-related complications such as hypertension, diabetes, and immobility, impacting her quality of life and healthcare costs. Focusing on obesity management as the capstone project is crucial due to its prevalence, healthcare resources, and general well-being. A study by Tiwari and Balasundaram (2023), summarizes obesity issues by loss of 6-7 years of life expectancy for people aged 40, higher risk of chronic disease development, discrimination in hiring and working (especially of females), and 30-40% higher medical cost than normal weight individuals. Obesity is highly relevant to my profession as a nurse because it requires comprehensive interventions, multidisciplinary care coordination, and the utilization of technological solutions to address obesity. This paper entails intervention solutions to improve her health outcomes cost-effectively and within regulatory guidelines.
Role of Leadership and Change Management
Sarah’s case is complex; leadership is important in enhancing outcomes by providing personalized care and enhancing satisfaction. Transformational leadership can guide healthcare providers to maximize health outcomes. It will enable healthcare personnel to tailor care plans according to Sarah’s needs and preferences. For instance, under transformational leadership, interdisciplinary teams, including physicians, nurses, dieticians, and psychologists, develop customized care management plans for Sarah’s obesity and its related health management. Discussion among the interdisciplinary team will enable evaluate the risk factors associated with Sarah’s health and share their perspective for customizing treatment plans addressing obesity from multiple perspectives, enhancing patient satisfaction (Specchia et al., 2021).
Transformational leaders can train interdisciplinary teams to develop collaboration and knowledge essential for obesity management, emphasizing a compassionate and empathetic approach toward the patients. A nurse leader can play a significant role by communicating among healthcare professionals and educating patients thoroughly about the treatment plan to improve health outcomes and enhance patient experience. Resultantly, transformational leadership motivates staff members to manage obesity and its related concerns through updated knowledge and evidence-based practices, inspiring shared decision-making (Specchia et al., 2021).

Change management in a healthcare organization also plays a significant role in managing obesity and its related health concerns (Parker et al., 2022). Regarding Sarah’s case, Lewin’s change model effectively implements strategies that have long-lasting impacts and ensure the prevention of obesity and other non-communicable disease like hypertension and diabetes. The three stages of the change model include Unfreezing; it involves dismantling the practices that are no longer effective in obesity management, like fragmented care, where each healthcare discipline works individually. Collaborating is essential for managing obesity so Sarah can have a quality life and enhanced experience.
The next stage is Change, and it involves evidence-based practices like open communication and technological integration for collaboration among nurses, patients, physicians, and other necessary healthcare professionals. Lastly, the freezing stage comes, where evidence-based practices are frozen through authoritative policies that are essential and effective for Sarah’s obesity management (Parker et al., 2022).
The Proposed Intervention
An effective intervention for Sarah would be a series of educational sessions delivered through telehealth, such as video conferencing. These sessions would focus on nutrition education and physical activity guidance tailored to her mobility limitations. The session will incorporate behavioral strategies to promote sustainable lifestyle changes for managing her obesity and related health issues effectively (Amiri et al., 2020).
Nursing Ethics in Developing the Proposed Intervention
Nursing ethics have a substantial role in developing the proposed intervention for Sarah’s obesity and related health issues. Ethical considerations guide nurses in providing patient-centered care, respecting Sarah’s autonomy, and ensuring beneficence and non-maleficence in the intervention process. Nurses must uphold ethical principles such as integrity, honesty, and confidentiality throughout the educational sessions delivered via telehealth. The principles foster trust and collaboration with Sarah to achieve positive health outcomes while respecting her dignity and rights as a patient (Martinelli et al., 2023).
Strategies for Communicating and Collaborating with Patients
Communication and collaboration with patients are important aspects of obesity management. The language sets the tone for communication between healthcare professionals, including nurses and patients. For that, plain, compassionate, and empathetic language is essential to make Sarah comfortable discussing weight. A study by Auckburally et al. (2021), highlights the importance of using non-stigmatizing language to communicate with obese patients and states the positive response by patients through active engagement and consistent adherence to treatment plans.
This approach helped me communicate risk factors and treatment options, discuss challenges, and develop a treatment plan for Sarah to promote compliance and sustain obesity management goals. Collaboration strategies are also meaningful in effective patient outcomes for obesity management. Obesity and its related health issues are the target of bias within healthcare and society, ultimately becoming the reason for fragmented care and demotivation. Evidence-based Motivational Interview (MI) is a means of interacting with obese patients with core values of empathy and discrepancy, constructively addressing resistance and self-efficacy (Olateju et al., 2021).

This allows healthcare professionals to think from the patient’s perspective, educating the patient about the reason for the change and addressing resistance due to dilemmas, fear of the unknown, rebellion, and self-belief. Based on the literature, we incorporated open-ended questions with Sarah to facilitate dialogue and the affirmation technique to replace negative attitudes with positivity and encourage persistence. Reflective listening was another approach to enhance patient satisfaction by navigating the pathway of managing obesity and enhancing experience via collaboration (Olateju et al., 2021). Moreover, input from patients has significant benefits. For instance, patient input or feedback makes patient-centered care effective by communicating their response to treatment plans.
The input also allows patient participation in communicating cultural beliefs, values, and preferred language, enhancing understanding of healthcare issues and concerns. Healthcare professionals can address such concerns, facilitating effective health outcomes during obesity management intervention (Han et al., 2023). It is ethically correct as it safeguards the autonomy principle of ethics and demonstrates a shared decision-making approach. Patient input can enhance patient experience by communicating confidently, developing trust, and seeking care with the least medical errors (Han et al., 2023). We ensure Sarah articulates her issues without hesitation and communicates them regarding any required support for obesity management.
Standards of Nursing Practice and Government Policies
Practicing standards and policies have guided nurses to make informed and personalized plans for managing Sarah’s obesity problem. Regarding this, The American Nurse Association (ANA) helped me make plans within ethical standards, facilitating care coordination and shared decision-making (Haddad & Geiger, 2020). It allowed us to respect Sarah’s rights while proposing a plan, ensuring her well-being through nutritional, physical, and behavioral interventions.
The Affordable Care Act (ACA) is a policy for improving accessibility to healthcare services and insurance coverage (Rdesinski et al., 2023). It helped propose obesity management intervention for Sarah by availing financial coverage options to treat health concerns effectively. Health Insurance Portability and Accountability Act (HIPAA) assisted in proposing intervention for Sarah. HIPAA compliance ensures patient information is secured and kept confidential while communicating among healthcare teams (Edemekong et al., 2020).
The effectiveness of these policies can be assessed from multiple perspectives. For instance, following ANA standards of ethical practices, Sarah will receive comprehensive care. The care plan would improve health outcomes through weight management, nutrition, physical activity, and behavioral health (Haddad & Geiger, 2020). ACA will also make intervention effective, improving accessibility to healthcare services. Sarah will be able to receive care timely with regular check-ups. She can cover finances essential for nutritional counseling, physical therapy, and behavioral change. Lastly, confidentiality is crucial for fostering open communication between the patient (Sarah) and healthcare providers, enabling collaborative intervention for obesity management under HIPAA compliance (Edemekong et al., 2020).
Enhancing Quality of Care, Promoting Safety, Minimizing Costs
Implementing educational intervention, which includes nutritional education, physical activity, and behavioral change strategies through established benchmarks of BMI. The World Health Organization (WHO) has established tools for measuring obesity and managing weight based on Body Mass Index (BMI). For obese patients, the BMI is divided into three classes: the higher and the worse. For instance, class I (30.0–34.9), class II (35.0–39.0), and class III (≥40.0) (Weir & Jan, 2023).
Based on this, Sarah’s nutritional educational session includes food that will reduce her BMI within the normal range and ensure her diabetes and hypertension are in control. A balanced macronutrient diet, fiber-rich food, reduced sodium intake, and monitoring of potassium in the body are part of the educational sessions established through the DASH diet and are effective for patients’ obesity and hypertension management (Altawili et al., 2023). Similarly, Waist Circumference is another tool that targets mass accumulation in the central body, significantly impacting cardiovascular health.

The interventions also ensure the safety of patients; for instance, a study highlights that each unit’s decrease in BMI reduces the chances of complications like hypertension, diabetes complications, and mobility issues. Physical exercises in educational sessions will reduce the risk of mobility-related concerns like falling, hence ensuring patient safety (Ross et al., 2020). Moreover, educational sessions via telehealth will assist Sarah in self-management through healthy food, more physical activity, and behavioral change. It is associated with reduced hospitalizations, saving travel costs and complications, hence reducing the costs associated with treating obesity-related issues, as obese patients spend 30-40% more than their peers with a healthy weight (Sunol et al., 2023; Tiwari & Balasundaram, 2023).
The National Institutes of Health (NIH) states that decreasing waist circumference from a certain threshold will lower cardiovascular risk for patients. These ensure that Sarah’s quality of life is improved through weight management. It is established that educational sessions have a positive impact due to learning about diet and physical activity without reinforcement. In particular, telehealth means to educate patients about obesity and its impact, and measurement tools pose further positive results due to cost-effective and continued care provision via follow-ups and remote monitoring (Ross et al., 2020).
Role of Technology, Coordinated Care, and Community Resources
Telehealth services can be crucial in Sarah’s case, providing remote monitoring, virtual consultations, and education sessions. This allowed us to address both concerns of immobility and cost, as Sarah was unable to pay frequent visits to the hospital and also learn about her treatment plan. Studies highlight the benefits of telehealth interventions in managing conditions like obesity, hypertension, and diabetes, improving outcomes and access to healthcare services (Xiao & Han, 2022). Implementing a care coordination approach involving a multidisciplinary team can significantly benefit Sarah. This approach ensures that all healthcare providers collaborate effectively, share information, and develop a personalized plan for managing obesity. A study encourages a coordinated approach for chronic disease management as it enhances patient experience, satisfies them, and improves treatment plan adherence (Foo et al., 2023).
This means Sarah will better adhere to nutritional guidance, physical exercises, and behavioral change recommendations with collaboration among healthcare providers, enhancing outcomes. Leveraging community resources such as support groups like the Community Preventive Services Task Force (CPSTF) and programs like Shape Up can enhance Sarah’s holistic care. Literature shows that community-based interventions are effective in managing obesity, promoting health literacy and healthy behavior, and improving well-being (Zeb et al., 2024). These resources will help Sarah access physical activity programs and structured health education and maintain a healthy lifestyle through emotional and behavioral support.
Conclusion
In conclusion, the proposed intervention for Sarah’s obesity and related health issues incorporates evidence-based educational sessions delivered via telehealth, guided by ethical principles and nursing standards. This intervention aims to improve Sarah’s care standard and safety and minimize medical costs through leadership, change management, and patient-centered communication strategies. By leveraging technology, coordinated care, and community resources, we can achieve holistic and effective obesity management for Sarah, ultimately improving her health and well-being.
